At Texas Community Counseling, we are committed to transparency, client rights, and ethical care. Below you will find information on how to access your records, contact our licensing board, and file a complaint if needed.

Request Your Health Records

You have the right to access your health records.

To request your records:

  1. Submit a written request to Texas Community Counseling

  2. Include your full name, date of birth, and contact information

  3. Specify whether you would like:

    • A full copy of your record, or

    • A summary
